All coaches and team managers are required by MYSA to submit an online Risk Management/Background check prior to interaction with club players.  Please check with MYSA for updated instructions for using the system. 

"As a returning coach, asst. coach or team manager, do I have to submit the online Background Check each soccer season?"


At the start of a FALL Soccer Season:
ALL Coaches, Asst. Coaches and Team Managers MUST comply with MYSA and submit a mandatory Risk Management/Background check.
At the start of a Summer Soccer Season:
Coaches, Asst. Coaches and Team Managers that submitted a background check nine months prior for the FALL soccer season are NOT required to submit another background check for the Summer season.  Your Fall background check compliance is valid through the following Summer season.

However, If you did NOT coach last fall season, you MUST comply with MYSA and submit a mandatory background check for the current summer season.
MYSA Medical Release (typable)

"When are NEW Medical Release forms required for players, and does the Medical form need to be notarized?"

At the start of a Fall Soccer Season:
ALL U9-U14 Fall players are required to submit a NEW MYSA Medical Release (typable) and provide the following documents to your team manager:

(a) Completed, signed and dated Medical Release form (Note: This form will also be valid for the upcoming Summer season, nine months from the time the form is signed in the Fall.)
(b) Photocopy of your birth certificate
(c) Current headshot (photo) for your MYSA Player Pass.
At the start of a Summer Soccer Season:
For those who played Fall soccer, check with your Fall manager to get your Medical Release form and birth certificate.  However, PLEASE REVIEW the information on your Fall Medical Release form to make sure the information is current before submitting it to your new summer manager.

If you are a U15 or older player, or you did not play soccer last fall, then you are required to complete, sign and date a NEW Medical Release form for the Summer season.  Please p
rovide the following documents to your Summer team manager:  (a) Signed and completed Medical Release form, (b) a photocopy of your birth certificate, and (c) a current headshot for your Summer Player Pass.
Does the Medical Release form need to be notarized?
  • A Medical Release form must be notarized if your team will be playing in an out-of-state tournament, or the team will potentially be playing in State Cup. (State Cup does not apply to the U9-U12 age brackets.) Please refer to the MYSA Medical Release form for more information, or check with your team manager.
  • A player's Medical Release form does NOT have to be notarized if the team is not playing in an out-of-state tournament and/or your team does not to play in State Cup by choice or age restriction (see above.)
