REQUIREMENTS: Please check in at least one hour prior to your first game: Dakota REV requires the following documentation for each Team, Player and Guest Player at the time of Team Check-In: • (For non-MN or WI teams only) State Association approved Application to Travel Form
• MYSA Match Report Form One copy will be collected at registration. Copies NOT required for each game
• Guest Player Form, if necessary, with Max. 5 guest players per team
• MYSA Medical Release Form
• Current USYSA Player Passes for players and guests signed and laminated. No passes, No play, No exceptions!
• Teams with unpaid registration fees will not be registered
TOURNAMENT T-SHIRTS
Long-sleeve Black T-shirts with a Neon Lime Green logo design will be for sale at tournament headquarters. Click HERE to see the 2010 T-shirt Design. Pre-orders were due Friday, April 23rd. Team managers that placed pre-orders can pick up their shirts at the time of team check-in.
CONCESSIONS
Stop by Tournament Headquarters during tournament hours for a grilled burger, hot dog, chips, candy, soda, Gatorade, water, coffee, hot chocolate, and more. During Weekend I, concessions will also be available at the Dakota County Technical College (DCTC) field location.
TOURNAMENT RAFFLE This year's Tournament Raffle is better than ever. At the time of Team Check-in, Managers will receive a string of tickets, one ticket for each registered player. Please assign each ticket to a player on your roster. The raffle drawing will take place throughout the tournament weekends and the winning ticket numbers will be posted at Tournament Headquarters and online. Check back to see if your ticket is a winner
Winners please claim your prizes anytime during Weekend II at Tournament Headquarters in Apple Valley.
