Provide the following to the center referee at the beginning of each game/player check-in:

1. Competed MYSA Match Report
2. Coach and Member Passes — Center ref will hold your passes during the game, so don’t forget to ask for them back at the end of the game
3. #10 Mailing Envelope —  Including affixed postage stamp, “neon” mailing label to district representative, and be sure to include your return address
4. Game Ball — Coach provides proper size, inflated, etc
5. Have your binder with Medical Release Forms with you at each game
6. Corner flags
7. First Aid Kit

Referrees for REV "HOME" Games
For REV Home games, the refs should be at the field 30 minutes prior to kick off.  If the refs are NOT at the field 25 minutes before scheduled kick off, please call or text Mike Guillou at 612-709-0137.  For Away games, the opposing team is responsible for the referees.

You will provide the following to the center referee prior to the start of player check-in:

3. Have your binder with Medical Release Forms with you at each game
4. First Aid Kit

Save yourself last minute chaos by preparing and printing ALL of your MYSA Match Reports and Home Game Envelopes now. You can prepare your Match reports any time after you receive your final game schedule, or print them now leaving the game day information blank and hand-write in your game day information after you receive your final schedule:

Go to the "typable" MYSA Match Report

Start by typing in the "repetitive" information that pertains to ALL games:

1. Players' names (Full names, as they appear on their Member Pass)
2. Registration Number (No. on each player's Member Pass)
3. Jersey Number
4. Ignore columns: Caution and Send-off
5. Include any Play-ups (you will need their Registration number from their Pass and Jersey number, Play up numbers cannot duplicate a number on your roster)
6. Fill in the "variable" information for your first game and print that page
7. Repeat Step 6 for your 2nd game and print, 3rd game and print....etc.
8. Continue until you have completed all Match Reports for the season.
9. Remember to have a pass-holding Team Leader (coach or manager) sign the Match Report before handing it over to the ref.

Prepare all of your "Home Game" envelopes

1. Write the game date on the lower corner of each envelope
2. Affix the MYSA mailing labels and a postage stamp.
3. Inside each envelope include: Dated and Signed MYSA Match Report.
4. Be sure to double-check your Match Report before handing it to the referee. 

As soon as possible following each Home and Away game, log into MYSA's website to report the final game score.  

In your MYSA packet from the Team Rep. Meeting you will receive instructions on how to log into the MYSA website and report league game scores using a unique user name/password.  Bookmark this page as you will return to it throughout the season.
